Information
The Team
At your reassessment a
interdisciplinary health team (IDT) will be present to discuss any
concerns and to review your general status. Your IDT will include
a:

Physiotherapist (PT)
-
Occupational Therapist
(OT)
-
Nurse (RN or Clinical
Nurse Specialist)
-
Spinal Rehabilitation
Specialist (Doctor)
One of these team members will be allocated as your
Key
Worker (KW), who will generally "chair" the assessment to
ensure that relevant details are covered and be available as a
contact should you have any questions following the
team assessment.

Social Worker (SW)
A Social Worker is available on request. Please let us know
if you would like our Social Worker to be part of the Team.
You are welcome to bring a support
person with you.
What to expect
The purpose of the team assessment
is to answer any questions you may have and to help problem solve
any issues or complications that may have arisen as a result of your
spinal injury.
A medical check-up is part of the
team assessment, where the team may wish to examine your skin, check
your muscle function and sensation, and evaluate your posture to
check for any changes that may lead to problems in the future.
In order to carry out these
assessments we may require you to be examined on a bed. (If you use
a hoist for transfers please bring your hoist sling with you)
You may have been sent
forms for ultrasound, blood and urine tests depending on
individual need. If so please complete as soon as possible
so that these can be discussed with you at your appointment.
